To be eligible for a group plan, the
EMPLOYER
must...

Have at least TWO - Full Time -
W2 employees working at least 30 hours/week
(Husband and Wife as only employees does not qualify, must have at

one other employee)

Maintain at least Two employees at all times (and
the policy cancels if you drop to one employee) |

Meet the minimum participation rules
(100% participation of 1-4 employee groups, 75%
of 5-20 employees, etc) |

Pay the monthly premium from the Employer Funds |

Pay at least 50% of the single employee rate from
Employer Funds (can pay more, but this is the
minimum) |

Employees can waive coverage if they have credible GROUP coverage
through a spouse or family member |
